Permanent Resident Alien (PRA) Verification Form
All PRA applicants to The University of Tulsa must have their status verified and documented by either an
official representative of the university’s Division of Continuing Education or a notary public.
(Check your current school or telephone directory for a notary public near you.)
Verification consists of this completed form and a signed photocopy of both sides of your PRA card (“Green card”).
Complete the student section of this form, then obtain the necessary verification. Return the completed form to:
